The providers at Barbara Fordyce & Associates, LLC are credentialed with many insurance companies such as Aetna, Anthem BC/BS, Aultcare, Cigna, Medical Mutual, Medicare, Optum and United HealthCare. Please contact your individual insurance policy to verify coverage for mental health services. Our friendly office staff can help answer questions regarding billing, and as a courtesy, can help with verification of benefits. Our office will submit billing to the client's insurance company. Any copays, coinsurances or deductibles are due at the time of service. It is ultimately the client's responsibility to understand their health insurance benefits and limitations. Any balances on invoices or non-covered services are the responsibility of the client. Further, clients are responsible for informing the office of any changes in insurance coverage and coordination of benefits if there are secondary insurances.
We also accept self-pay clients with payment due at the time of service.
Clients who do not have or do not use their health insurance have the right to a "Good Faith Estimate." The “Good Faith Estimate” explains how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. Payments can be made in-office, by calling the office, or through our secure client portal. We accept cash, check, VISA, Mastercard and American Express.
