Beginning services with our practice is simple. New clients may contact our office directly by phone or reach out online using the secure message form below. Our administrative team will help verify your insurance benefits, answer any questions, and match you with a therapist who fits your needs.

Billing Information
The licensed providers at Barbara Fordyce & Associates are in-network with many insurance companies such as:

Please contact your individual insurance policy to verify coverage for mental health services.
We also accept self-pay clients with payment due at the time of service.
Fees:
Initial Assessment: $225
1-Hour Session: $200
45-Minute Session: $135
Our friendly office staff can help answer questions regarding billing, and as a courtesy, can help with verification of benefits. Our office will submit claims to your insurance company. Any copays, coinsurances or deductibles are due at the time of service. Clients are responsible for any non-covered services. Further, clients are responsible for informing the office of any changes in insurance coverage and coordination of benefits if there are secondary insurances.
Payments can be made in-office, by calling the office, or through our secure client portal. Receipts and/or superbills can be provided as needed for HSA/FSA or out-of-network insurance claims.
We accept cash, check, VISA, Mastercard and American Express.

Clients can access our secure client portal to complete initial intake paperwork, view invoices, make payments, and send messages. Contact our office at 330-492-2006 to set up an initial appointment or use the contact form above.
Once you are established as a client, you will login to the client portal with the email you have provided us.

Good Faith Estimate
Clients who do not have or do not use their health insurance have the right to a "Good Faith Estimate." The “Good Faith Estimate” explains how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
